L’ecografia dinamica nella valutazione delle ostruzioni delle alte vie escretrici: utilità ed attendibilità

Riassunto – L’ecografia renale non è un esame di tipo funzionale e questo svantaggio comporta il fatto di non poter essere utilizzata per determinare il grado di ostruzione dell’alta via escretrice, per cui bisogna necessariamente ricorrere ad esami più o meno invasivi quali il test di Whitaker, renogramma e/o urografia. L’ecografia dinamica con test alla furosemide, oltre al dato morfologico, dà informazioni sul grado di ostruzione ed è molto utile nella valutazione pre e post-operatoria dei pazienti con patologie dell’alta via escretrice. Sono stati studiati 39 pazienti per complessive 72 unità renoureterali. Esclusi i pazienti con quadro ecografico "dinamico" di normalità, tutti gli altri sono stati anche studiati con renogramma e/o urografia. Dall’analisi dei dati emerge che l’ecografia dinamica ha una attendibilità sovrapponibile agli esami più invasivi sopracitati (88-95%). Per tali motivi, data la facilità d’uso e la sua disponibilità presso tutte le strutture, riteniamo che l’ecografia dinamica rappresenti un esame da impiegare in prima istanza nello studio delle ostruzione delle alte vie escretrici. Abstract – Renal ultrasound is not a functional type of examination, which means it cannot be used to determine the degree of obstruction of the upper urinary tract, for which more or less invasive exams such as Whitaker’s test, renogram and/or urography must be perfomed.Dynamic ultrasound with furosemide test gives information on the morphology and also the degree of obstruction and is extremely useful in the pre and post-operative assessment of patients with upper urinary tract pathologies. A total of 72 renoureteral units in 39patients were studied. Patients with a normal "dynamic" ultrasound picture were excluded, while the others were also studied with renogram and/or urography. Data analysis shows dynamics ultrasound reliability is on a par with that of the above-mentioned invasive methods (88-95%).Given the simplicity of use and its availability in all hospital structures, it is therefore considered that dynamic ultrasound should be used immediately in studying obstructions of the upper urinary tracts.
