grado e margini chirurgici positivi correlano con il grado dinteressamento
linfonodale, e quantificare le differenze prognostiche fra i pazienti N1 ed N2, dopo
prostatectomia radicale con linfoadenectomia. Abstract The aim of this study is to check if stage, grade and positive surgical margins are correlated with the degree of lymph node involvement and to quantify the prognostic differences between N1 and N2 patients after radical prostatectomy with lymphadenectomy. Of the 335 patients subjected to radical prostatectomy for prostate cancer, 60 had lymph node involvement: 35 (58.3%) N1 and 25 (41.7%) N2. The neoplasms N1 and N2 were compared for stage, grade and the presence of positive surgical margins. The specific-cause survival and biochemical recurrence of disease in 39 patients was assessed (follow-up of 40.6 months). Patients with N2 neoplasm had a worse prognosis than those with N1 neoplasm in relation to both biochemical recurrence and correlated death. All deaths and 80% of recurrences occurred in patients with neoplasms in pathological stage above pT3a and grade between 8 and 10.
